Addiction rarely occurs in a vacuum. For many, the struggle with substance use is deeply tied to underlying mental health conditions. This complexity requires specialized care known as dual diagnosis treatment, which addresses both the addiction and the psychological factors driving it simultaneously.

At Coastal Detox of Southern California, our dual diagnosis treatment program uses an integrated approach so that addiction and mental health conditions are treated together. Patients receive education to understand their diagnoses, expert clinical counseling to process difficult feelings and emotions, and practical applicable skills to help them lead rich, fulfilling lives after rehab.

If you or a loved one are seeking dual diagnosis care for your mental health and drug or alcohol addiction, we’re here to help you take the next step. Call our admissions team to learn more about our treatment, verify your insurance coverage, and start your intake today.

Understanding Dual Diagnosis Treatment

Dual diagnosis, also referred to as co-occurring disorders, describes the simultaneous presence of a substance use disorder (SUD) and one or more mental health conditions.

The primary goal of dual diagnosis treatment is to address the complex interplay between these conditions rather than treating them as separate, isolated issues. When a person receives integrated care, the same clinical team manages both disorders within a unified program framework with shared goals.

Why Integrated Care is Essential

  • Breaks the Cycle of Self-Medication: Many people use drugs and alcohol to numb symptoms of mental health disorders, such as anxiety or depression. Integrated care addresses these symptoms directly, reducing the urge to self-medicate.
  • Ensures Accurate Diagnosis: Clinicians can more accurately distinguish between primary mental health symptoms and those caused by substance intoxication or withdrawal.
  • Improves Long-Term Outcomes: Studies indicate that integrated care leads to higher rates of sustained sobriety and an improved overall quality of life.
  • Consistent Skill Building: Coping mechanisms learned in therapy, such as emotional regulation, can be applied to both mental health triggers and relapse prevention.

Common Co-Occurring Mental Health Disorders

While addiction can occur alongside nearly any psychiatric condition, certain disorders are more frequently diagnosed in dual diagnosis settings.

  • Depression & Mood Disorders: People may turn to drugs or alcohol to alleviate persistent sadness or hopelessness, which can ultimately worsen the condition by disrupting brain chemistry long term.
  • Anxiety Disorders: Substances like alcohol, which is a central nervous system depressant, are often used to quiet racing thoughts or manage social fear, leading to a cycle of dependency.
  • PTSD: Trauma survivors may use opioids or alcohol to numb painful memories and regulate hypervigilance.
  • Bipolar Disorder: Extreme mood swings may lead people to misuse substances in an attempt to self-regulate manic or depressive states.
  • ADHD: Challenges with impulse control and executive function can increase the risk of developing an addiction. Use of stimulant ADHD prescription drugs such as Adderall are increasingly common substances of abuse and addiction.
  • Personality Disorders: Conditions like Borderline Personality Disorder involve emotional instability, which significantly raises the risk of severe substance use.

Key Components of Effective Care

Effective dual diagnosis treatment utilizes a multi-pronged approach to stabilize the patient and provide them with lasting tools for recovery.

  1. Medical Detoxification: Safely managing withdrawal symptoms under the supervision of medical professionals to prepare the individual for therapy.
  2. Evidence-Based Psychotherapy: Utilizing therapies such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) to teach coping skills for both cravings and mental health symptoms.
  3. Medication Management: Oversight by a psychiatrist to stabilize mental health symptoms with appropriate medications that do not interfere with the recovery process.
  4. Expressive and Holistic Therapies: Incorporating music, art, and mindfulness to help patients process trauma and emotions without the pressure of finding the “right words”.

Phil Castagnola

Case Manager

Philip Castagnola, SUDCC, is the Program Director for Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Phil has over 15 years of continuous sobriety and is an active Sponsor in the San Diego Alcoholics Anonymous (AA) community. He struggled with substance abuse issues throughout his youth. He started using drugs at the age of 15, having been raised in an environment that normalized heavy drug and alcohol use. He spent a lot of time in and out of various rehab programs before being incarcerated in federal prison, which is where he finally completed an 18-month treatment program and used that time to work the 12 Steps.

After completing his sentence and serving his debt to society, he vowed to continue to give back to others who are going through similar hardships. Phil is passionated about sharing his experience with openness and honesty so that others may find hope for their future and find the strength to break free from the bondage of addiction.

In addition to a successful career as a certified counselor, Phil is also reconnecting with his passion for surfing. Phil surfed professionally in competitions for a few years in his early 20s and even owned his own surf shop (Select Surf Shop) before losing everything to his addiction. Now in his sobriety he is able to once again surf recreationally and in contests, showing that it’s never too late to reclaim your own narrative, overcome substance use disorder, and pursue the life you were always meant to have.

Jeff Swem, SUDCC

Director of Operations

Jeff Swem, SUDCC, is a Substance Use Disorder Certified Counselor and the Director of Operations at Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Jeff has 6 years of continuous sobriety and is active in the San Diego Alcoholics Anonymous (AA) community. Jeff struggled with substance abuse from a young age and endured some harrowing, near-death experiences before “settling down” into a career as a professional beer brewer. It was during the COVID outbreak that he surrendered to his higher power and made the choice to get sober for good.

After completing an inpatient rehab program and IOP aftercare, Jeff quit the brewing industry and pivoted to become a Substance Use Disorder Certified Counselor so he could surround himself with a caring and supportive sober community and help others who struggle with addiction by leading through example and sharing his personal experience.

Born and raised in San Diego, Jeff is married to his wife of 25 years. They have a son and daughter together, and thanks to the 12 Steps and a successful career at Coastal Detox, Jeff is able to be fully present for them every day, be a positive role model, and show them how it is possible to thrive in recovery.
