Acute fentanyl withdrawal symptoms typically begin within 6 to 24 hours after the last dose, reaching peak intensity within the first few days. While the most severe physical symptoms generally subside within 5 to 10 days, the exact duration varies depending on the person’s level of dependence and history of use.

Because fentanyl is a short-acting and highly potent synthetic opioid, the onset of withdrawal is often more rapid and intense than that of other opioids. Fentanyl is commonly found in other street drugs and counterfeit pills, further complicating the withdrawal symptoms and timelines for other substances.

If you or a loved one are struggling with fentanyl dependence, contact Coastal Detox today. Our team is here to help you safely manage withdrawal and begin your path to recovery.

Understanding Fentanyl Addiction and Dependence

Fentanyl is a synthetic opioid estimated to be 50 to 100 times more potent than morphine. This extreme potency facilitates the rapid development of physical dependence, a physiological state where the brain adjusts its chemistry to the constant presence of the drug. When dependence is established, the body requires the substance to maintain normal function. Suddenly reducing or stopping fentanyl use can cause a range of moderate to severe withdrawal symptoms.

Addiction, or opioid use disorder (OUD), is a chronic disease characterized by compulsive drug seeking despite harmful consequences. This condition involves long-term changes to brain function related to reward, motivation, and memory. Due to the high risk of accidental overdose from illegally manufactured fentanyl—often found in counterfeit pills or mixed with other illicit substances—understanding the transition from dependence to recovery is critical for successfully treating fentanyl addiction.

Fentanyl Withdrawal Symptoms

Fentanyl withdrawal is characterized by significant physical and psychological symptoms. The most common fentanyl withdrawal symptoms include:

  • Physical Pain: Severe muscle and bone aches, uncontrollable leg movements.
  • Gastrointestinal Distress: Intense nausea, vomiting, and diarrhea.
  • Nervous System Disruption: Chills, excessive sweating, hot and/or cold flashes, goosebumps.
  • Autonomic Responses: Increased heart rate and elevated blood pressure.
  • Sleep and Psychological Disturbances: Insomnia, restlessness, anxiety, and dysphoria.

Fentanyl Withdrawal Timeline

The withdrawal process generally follows a predictable clinical trajectory, though individual experiences are always different.

Phase

Timeframe

Primary Characteristics

Early Onset

6–24 Hours

Rapid appearance of anxiety, restlessness, and initial physical discomfort.

Peak Intensity

1–3 Days

Maximum severity of physical symptoms, including gastrointestinal issues and severe aches.

Acute Intensity

4–10 Days

Gradual decline of physical symptoms; persistent fatigue and sleep disturbances may remain.

Post-Acute

Weeks–Months

Potential for lingering psychological symptoms and intense cravings.

While this timeline outlines the typical progression of fentanyl withdrawal, individual experiences vary significantly. Factors that contribute to these differences include the duration and frequency of fentanyl use, the average dose consumed, and the method of administration.

The length and severity of fentanyl withdrawal symptoms also depend on the person’s overall physical health, metabolic rate, and the presence of any co-occurring physical or mental health conditions. Professional medical assessment is essential to determine the appropriate level of care and support based on these unique patient factors.

Seek Professional Medical Detox

Attempting to detox from fentanyl at home, either through tapering the dosage or quitting cold turkey, carries significant risks. The intensity of the withdrawal symptoms frequently leads to relapse within the first 24 hours. People often return to fentanyl usage simply to alleviate their physical pain.

Medical detox in a professional facility provides a secure, monitored environment where licensed medical teams can manage withdrawal symptoms safely and keep patients as comfortable as possible during the fentanyl withdrawal timeline.

Clinical settings, such as Coastal Detox of Southern California, utilize FDA-approved medications and evidence-based therapies to mitigate discomfort and monitor vital signs 24 hours a day. This professional support is the safest and most effective first step toward long-term recovery and the successful management of opioid use disorder.

Phil Castagnola

Case Manager

Philip Castagnola, SUDCC, is the Program Director for Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Phil has over 15 years of continuous sobriety and is an active Sponsor in the San Diego Alcoholics Anonymous (AA) community. He struggled with substance abuse issues throughout his youth. He started using drugs at the age of 15, having been raised in an environment that normalized heavy drug and alcohol use. He spent a lot of time in and out of various rehab programs before being incarcerated in federal prison, which is where he finally completed an 18-month treatment program and used that time to work the 12 Steps.

After completing his sentence and serving his debt to society, he vowed to continue to give back to others who are going through similar hardships. Phil is passionated about sharing his experience with openness and honesty so that others may find hope for their future and find the strength to break free from the bondage of addiction.

In addition to a successful career as a certified counselor, Phil is also reconnecting with his passion for surfing. Phil surfed professionally in competitions for a few years in his early 20s and even owned his own surf shop (Select Surf Shop) before losing everything to his addiction. Now in his sobriety he is able to once again surf recreationally and in contests, showing that it’s never too late to reclaim your own narrative, overcome substance use disorder, and pursue the life you were always meant to have.

Jeff Swem, SUDCC

Director of Operations

Jeff Swem, SUDCC, is a Substance Use Disorder Certified Counselor and the Director of Operations at Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Jeff has 6 years of continuous sobriety and is active in the San Diego Alcoholics Anonymous (AA) community. Jeff struggled with substance abuse from a young age and endured some harrowing, near-death experiences before “settling down” into a career as a professional beer brewer. It was during the COVID outbreak that he surrendered to his higher power and made the choice to get sober for good.

After completing an inpatient rehab program and IOP aftercare, Jeff quit the brewing industry and pivoted to become a Substance Use Disorder Certified Counselor so he could surround himself with a caring and supportive sober community and help others who struggle with addiction by leading through example and sharing his personal experience.

Born and raised in San Diego, Jeff is married to his wife of 25 years. They have a son and daughter together, and thanks to the 12 Steps and a successful career at Coastal Detox, Jeff is able to be fully present for them every day, be a positive role model, and show them how it is possible to thrive in recovery.
