The decision to stop drinking alcohol is a common resolution for the new year. Whether you’re just doing a “dry January” or trying to get sober for good, quitting alcohol will help you lead a healthier, more balanced life.

For many people, the immediate impulse is to simply stop—to go “cold turkey.” While this approach sounds straightforward and perhaps even admirable in its determination, for anyone with moderate to severe alcohol use disorder (AUD), this approach actually has significant, life-threatening dangers.

The Risks of Alcohol Quitting Cold Turkey

Heavy alcohol use over time significantly impacts the daily operations of the brain and central nervous system. When a person drinks heavily and consistently, their body actually becomes dependent on alcohol to function normally.

Alcohol dependence occurs when the brain and central nervous system adapt to the constant presence of heavy alcohol, requiring continued use to prevent withdrawal symptoms.

When alcohol is suddenly removed from the system, as is the case when a person quits cold turkey, the body quickly enters alcohol withdrawal. The withdrawal process is not only uncomfortable but can be deadly. The severity of symptoms depends on the duration and quantity of alcohol consumption, but the risks are universal for those with physical dependence.

Alcohol Withdrawal Symptoms

  • Mild to Moderate: Anxiety, hand tremors (the “shakes”), nausea, vomiting, headache, insomnia, and profuse sweating.
  • Severe and Life-Threatening: Seizures and Delirium Tremens (DTs).

Delirium Tremens (DTs)

DTs are the most severe and dangerous symptom of alcohol withdrawal. They typically begin 48 to 96 hours after the last drink and are a medical emergency. DTs can cause:

  • Extreme confusion and disorientation.
  • Severe agitation.
  • Fever and profuse sweating.
  • Hallucinations (visual, auditory, or tactile).
  • Fatal complications, including severe dehydration, irregular heartbeat, and stroke.

Because of the risk of seizures and DTs, attempting to detox from alcohol without medical supervision is never recommended.

Medical Detox is Essential

A safe, successful recovery always begins with medically managed detox. This process ensures that your body stabilizes safely before you begin addressing the underlying causes of your drinking habits.

Trying to manage alcohol withdrawal alone often leads to relapse, as the intense discomfort and cravings become unbearable.

Why Choose Coastal Detox

Seeking help from the clinical experts at Coastal Detox of Southern California is the safest and most effective way to navigate the initial alcohol withdrawal phase. Our San Diego inpatient detox and rehab offers three critical advantages:

Medical Stabilization and Safety

Coastal Detox provides 24/7 medical monitoring by trained professionals. This supervision is vital for:

  • Symptom Management: Medications (such as benzodiazepines) are administered to reduce the risk of seizures and mitigate the severity of DTs and other symptoms.
  • Preventing Complications: Licensed medical staff are ready to intervene immediately if vital signs fluctuate or if severe complications arise.
  • Nutritional Support: Alcohol misuse can lead to severe nutritional deficiencies. We’ll make sure you stay properly hydrated and nourished through necessary supplements and our in-house chef who prepares fresh meals daily.
  • Community Support: Lean on a built-in support system with the other detox residents, in addition to our staff who are all in recovery themselves. At Coastal Detox you’ll find a tight-knit community in which to start your sobriety journey.
  •

A Foundation for Long-Term Recovery

Detox is just the first step. Coastal Detox starts you on the continuum of care needed for lasting sobriety.

  • Integrated Treatment: Once medically cleared, you seamlessly transition into therapeutic programs, including individual and group counseling, to explore the root causes of your alcohol use.
  • Relapse Prevention: Through evidence-based therapies, you’ll learn essential coping mechanisms, identify triggers, and develop a personalized plan to maintain sobriety after you leave our care.

A Supportive and Judgment-Free Environment

Attempting recovery at home can be overwhelming and isolating. Coastal Detox provides a structured, supportive environment that minimizes triggers and maximizes focus on healing. Being surrounded by professionals and peers who understand the challenges of alcohol addiction is invaluable during this vulnerable stage.

We’re Ready When You Are

The urge to quit alcohol “cold turkey” is understandable, but it is a potentially fatal risk. If you or a loved one is dependent on alcohol, the only responsible and safe path forward is through professional medical detoxification.

Phil Castagnola

Case Manager

Philip Castagnola, SUDCC, is the Program Director for Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Phil has over 15 years of continuous sobriety and is an active Sponsor in the San Diego Alcoholics Anonymous (AA) community. He struggled with substance abuse issues throughout his youth. He started using drugs at the age of 15, having been raised in an environment that normalized heavy drug and alcohol use. He spent a lot of time in and out of various rehab programs before being incarcerated in federal prison, which is where he finally completed an 18-month treatment program and used that time to work the 12 Steps.

After completing his sentence and serving his debt to society, he vowed to continue to give back to others who are going through similar hardships. Phil is passionated about sharing his experience with openness and honesty so that others may find hope for their future and find the strength to break free from the bondage of addiction.

In addition to a successful career as a certified counselor, Phil is also reconnecting with his passion for surfing. Phil surfed professionally in competitions for a few years in his early 20s and even owned his own surf shop (Select Surf Shop) before losing everything to his addiction. Now in his sobriety he is able to once again surf recreationally and in contests, showing that it’s never too late to reclaim your own narrative, overcome substance use disorder, and pursue the life you were always meant to have.

Jeff Swem, SUDCC

Director of Operations

Jeff Swem, SUDCC, is a Substance Use Disorder Certified Counselor and the Director of Operations at Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Jeff has 6 years of continuous sobriety and is active in the San Diego Alcoholics Anonymous (AA) community. Jeff struggled with substance abuse from a young age and endured some harrowing, near-death experiences before “settling down” into a career as a professional beer brewer. It was during the COVID outbreak that he surrendered to his higher power and made the choice to get sober for good.

After completing an inpatient rehab program and IOP aftercare, Jeff quit the brewing industry and pivoted to become a Substance Use Disorder Certified Counselor so he could surround himself with a caring and supportive sober community and help others who struggle with addiction by leading through example and sharing his personal experience.

Born and raised in San Diego, Jeff is married to his wife of 25 years. They have a son and daughter together, and thanks to the 12 Steps and a successful career at Coastal Detox, Jeff is able to be fully present for them every day, be a positive role model, and show them how it is possible to thrive in recovery.
