At Coastal Detox, we believe that true healing begins with radical honesty—both about the nature of addiction and the complex mental health conditions that often accompany it. One of the most frequently misunderstood topics in behavioral health is substance-induced mood disorder.

Patients often come to us wondering if their intense anxiety, depression, or mood swings are a sign of a primary mental health diagnosis or something else entirely. Understanding the role drugs and alcohol play in these symptoms is a vital step in crafting a successful recovery plan.

If you’re ready for individualized addiction treatment that addresses your unique needs and mental health condition, take the first step by calling our caring admissions team today

What is Substance-Induced Mood Disorder?

Substance-induced mood disorder is a condition where a person experiences significant disturbances in mood—episodes of depression, mania, or anxiety—that are directly caused by drug and/or alcohol use, intoxication or withdrawal.

Unlike a primary mood disorder (like Bipolar I or Major Depressive Disorder), which can co-occur with addiction or exist independently of any substance use, substance-induced mood disorder is directly correlated with the substance use itself

Signs and Symptoms of Substance-Induced Mood Disorder

Drugs and alcohol disrupt the brain’s natural regulation of serotonin, dopamine, and other neurotransmitters. Therefore, the symptoms of substance-induced mood disorder can be difficult to identify because they often present like other mental health conditions.

While every person’s experience is unique, symptoms generally fall into these categories:

  • Anxiety: Patients may experience intense feelings of restlessness, panic attacks, or persistent worry that coincide with the onset of withdrawal or substance use.
  • Depression: This involves a profound, heavy feeling of hopelessness, loss of interest in activities, fatigue, and persistent sadness that appears linked to the addiction cycle of tolerance, dependence, and withdrawal.
  • Bipolar-like Symptoms: This includes extreme and rapid fluctuations in energy, irritability, and mood, often shifting from high-energy agitation to low-energy despair as the substance leaves or enters the system.

How is Substance-Induced Mood Disorder Diagnosed?

Distinguishing between a primary mental health disorder and a substance-induced one is a critical medical task. At Coastal Detox, this diagnosis is part of a comprehensive clinical assessment. Our team looks at the timing of your symptoms:

  • Do they appear only during intoxication or withdrawal?
  • Do they persist during long periods of sobriety?

If symptoms subside once the substance is removed and the brain begins to stabilize, it strongly suggests a substance-induced origin rather than a primary psychiatric condition.

How Does Substance-Induced Mood Disorder Affect Treatment?

The presence of substance-induced mood disorder changes the landscape of addiction treatment, but it does not change our goal: helping you thrive.

When mood disorders are induced by substances, stabilization is the first priority. We focus on a safe, medically supervised detox to clear the substances from the system. Once the body is stabilized, the true underlying mental health needs become clearer, allowing us to pivot from immediate crisis management to long-term healing.

Following detox and the initial stabilization period, patients are routinely re-evaluated for signs of primary mental health conditions so they can receive the appropriate dual diagnosis care. Addiction therapy that also addresses co-occurring mental health disorders together in the same treatment program has significantly higher success rates and lower instances of relapse

Comprehensive Addiction Treatment

At Coastal Detox, we don’t just treat addiction; we treat the whole person. Comprehensive treatment for addiction with substance-induced mood disorder requires a dual approach:

  1. Clinical Stabilization: Ensuring the physical safety of our patients as they navigate the difficult early days of withdrawal.
  2. Holistic Therapies: We utilize expressive therapies like music and art, which provide a “backdoor” to process powerful emotions or trauma when traditional talk therapy feels overwhelming.
  3. Personalized Care: Our low patient-to-staff ratio ensures that our clinical team can tailor every treatment plan to the specific needs of the individual, not just the diagnosis.

Recovery is a journey of rediscovering who you are beneath the noise of addiction. If you are struggling, you don’t have to walk this path alone. Let our team of medical and clinical experts show you how to thrive in sobriety.

Phil Castagnola

Case Manager

Philip Castagnola, SUDCC, is the Program Director for Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Phil has over 15 years of continuous sobriety and is an active Sponsor in the San Diego Alcoholics Anonymous (AA) community. He struggled with substance abuse issues throughout his youth. He started using drugs at the age of 15, having been raised in an environment that normalized heavy drug and alcohol use. He spent a lot of time in and out of various rehab programs before being incarcerated in federal prison, which is where he finally completed an 18-month treatment program and used that time to work the 12 Steps.

After completing his sentence and serving his debt to society, he vowed to continue to give back to others who are going through similar hardships. Phil is passionated about sharing his experience with openness and honesty so that others may find hope for their future and find the strength to break free from the bondage of addiction.

In addition to a successful career as a certified counselor, Phil is also reconnecting with his passion for surfing. Phil surfed professionally in competitions for a few years in his early 20s and even owned his own surf shop (Select Surf Shop) before losing everything to his addiction. Now in his sobriety he is able to once again surf recreationally and in contests, showing that it’s never too late to reclaim your own narrative, overcome substance use disorder, and pursue the life you were always meant to have.

Jeff Swem, SUDCC

Director of Operations

Jeff Swem, SUDCC, is a Substance Use Disorder Certified Counselor and the Director of Operations at Coastal Detox of Southern California. He holds an Associate’s in Science and Certification in Alcohol and Other Drug Studies from San Diego City College.

Jeff has 6 years of continuous sobriety and is active in the San Diego Alcoholics Anonymous (AA) community. Jeff struggled with substance abuse from a young age and endured some harrowing, near-death experiences before “settling down” into a career as a professional beer brewer. It was during the COVID outbreak that he surrendered to his higher power and made the choice to get sober for good.

After completing an inpatient rehab program and IOP aftercare, Jeff quit the brewing industry and pivoted to become a Substance Use Disorder Certified Counselor so he could surround himself with a caring and supportive sober community and help others who struggle with addiction by leading through example and sharing his personal experience.

Born and raised in San Diego, Jeff is married to his wife of 25 years. They have a son and daughter together, and thanks to the 12 Steps and a successful career at Coastal Detox, Jeff is able to be fully present for them every day, be a positive role model, and show them how it is possible to thrive in recovery.
